Welcome to our FAQ section! Here, we have compiled a list of commonly asked questions to provide you with quick and helpful answers. General
Give us a call at 774-325-0574 or contact us through email. You must be seen by Pam Cavanaugh, PT for a PT evaluation prior to seeing Jaimee Lavin, PTA due to state license restrictions.
Shoreline Physical Therapy and Wellness now accepts Blue Cross Blue Shield, Mass General Brigham Health Plan (Commercial) and Medicare Part B for our services. Worker's compensation is not currently being accepted. For those patients that have exhausted their insurance benefits, or don't have physical therapy coverage, Shoreline PT offers a self-pay option. We can also offer this option for out of network patients, as well as patients with high deductibles. We can provide a Superbill for you to submit for reimbursement. However, reimbursement is not guaranteed. Each insurance company differs. Please contact your personal company prior to your visit to see if reimbursement is an option for you.
The state of Massachusetts does not require a doctor’s referral to access Physical Therapy services. However, your insurance company may require a referral for reimbursement.
You will receive a text or email confirmation of appointment date and time. There will be online intake forms prior to your appointment that will be linked in your Simple Practice patient portal.
Pamela Cavanaugh will perform a complete evaluation to identify your physical therapy diagnosis and build your personalized plan of care. She will identify how often and how long you may need Physical Therapy.
We travel to you, with the ability to treat in all environments. We can treat in your pool, home gym, on your boat, or even on the beach. A travel fee of up to $50 may apply.
Please wear comfortable, loose fit clothing.
Please remember to cancel and reschedule 24 hours in advance of your appointment time. Cancellations and re-scheduled sessions will be subject to a full charge if NOT RECEIVED AT LEAST 24 HOURS IN ADVANCE. This is necessary because a time commitment is made to you and is held exclusively for you. If you are late for a session, you may lose some of that session time.
We're proud to be TPI certified! TPI (Titleist Performance Institute) is the world's leading educational organization dedicated to the study of how the human body functions in relation to the golf swing. For over twenty years, TPI has gathered the world’s largest technical database of the game’s top Tour professionals as well as every-day golfers. TPI collects 3D motion capture, force, pressure, launch monitor, strength, power, and movement data on every player that visits the TPI campus. Using this data, TPI discovered how a properly functioning body allows a player to swing a golf club in the most efficient way possible. Conversely, TPI determined how physical limitations in a player’s body can adversely affect the golf swing and potentially lead to injury. This relationship is what TPI calls The Body-Swing Connection and is the foundation of all professional Certification courses offered by TPI.
